Dame Judi Dench once had a serious wardrobe malfunction which forced her to perform on television in her underwear.The then 23-year-old was completing a quick costume change on the set of her first ever TV role when she forgot to put on a skirt - and had to make do with just a blouse.Dench, now 72, says, "There was one shot where I had to run and do a change from one scene to another. When I got to the next scene I was standing on some stairs and I realised I hadn't got my skirt on."So I thought I'd sit down and the cameras were able to film me from the waist up. That's how on the ball the crew were".
