Vijay Singh was one shot ahead at the wind-swept Kapalua, but Appleby made two birdies on the last four holes to force a playoff.
Singh missed his birdie attempt from nine feet on the first extra hole.
Appleby holed his putt to join Gene Littler as the only players to win the tournament in three straight years.
He also became the 15th player in PGA Tour history to win the same event three straight years.
Appleby made hard work of the victory after starting the final round five shots clear of his Fijian opponent.
Despite strong winds, Singh holed a spectacular 7 under par 66, the weeks best round on the Kapalua Plantation course, which was 9 shots better than the average score in the final round.
Appleby won for the seventh time on the PGA Tour, earning $1.08 million and adding to his collection of Mercedes-Benz sports cars.
