As Ric arrives at work, Abra is sitting in reception. He says he has come to say goodbye after not getting the job at Holby, but Ric thinks he is being selfish and ungrateful as he walks away from Holby once more. On leaving the hospital Abra walks in front of an ambulance. Thankfully he has only sprained his wrist, but Ric takes him in to check him out.

On ITU, Alex Deacon, Maria’s birth mother, has stabilised. She still does not know who Maria is, but Sam helps Maria cope by taking over her treatment. She is suffering from polyps, which are non-malignant, but are causing internal bleeding. She needs a hysterectomy as soon as possible, but is against it, planning for the first time to have a baby with boyfriend of eight years Simon. He is keen for her to have the operation as he couldn’t bear to lose her, but she is insistent she will not go ahead with the operation after wanting a baby for so long.

A teenager who seems to be sleeping rough outside Holby gets into an argument with a boy and is stabbed outside the hospital. Abra and Ric witness the attack and go to help him. On examination Ric becomes concerned by his other injuries so asks Abra to help. The young man, Cassidy Walbeck, needs an urgent operation, but is more interested in finding his girlfriend Brogan, who is apparently in Holby somewhere. It transpires that it was her brother who had stabbed Cassidy, as he blamed Cassidy for a car accident Brogan had been injured in a few days previous. Cassidy is wanted by police for questioning as he was driving the car, but ran away leaving Brogan in a life and death situation.

Maria tries to talk to Simon and ease the situation between him and her birth mother. Simon thinks Alex is being selfish by not having the operation, but she continues to insist she wants a child. The situation upsets Maria, as she can’t understand how someone who wants a child so much gave her up at birth. Sam understands, but encourages her to move on as they have to try and help to save he life.

Lola is not impressed to see Abra in Holby again. She wants him out for good and out of Kyla’s way. However, Abra is proving useful to Ric, keeping Cassidy calm about his operation. The knife has punctured his large intestine and he needs a colostomy bag. As Ric prepares for the operation, Cassidy makes a run for it in hope of finding Brogan. Brogan is Elliot’s patient in ITU and has a pneumothorax.

Meanwhile, Kyla looks after a young boy named Daren who has been admitted with a fractured wrist, but seems to have been left unvisited by his mother. Eventually his mother turns up and both Lola and Kyla are suspicious of her. They decide Daren needs to stay in for more tests, as he has a low blood count; his mum happily leaves him there again and heads off having left her other four children in the car. Daren has not spoken a word, but he eventually starts to speak to Kyla. He reveals that he is fostered, but not allowed to speak about his foster mum. He talks about a ‘medicine’ his foster mum gets from the pub, and Kyla realises she is a drinker.

Maria decides to tell Alex that she knows that she has had a child previously from her medical notes. Alex reveals all to Maria saying how she did a terrible thing when she was young and gave her baby away. Alex now wants another child to make things right and stop the guilt she feels for what she did. As Maria is just about to tell her that she is her daughter, Simon interrupts and begs Alex to have the operation for him. She agrees and Maria leaves them to talk.

On seeing Brogan, Cassidy is shocked by her current state, but Kyla and Abra find him and take him back to ITU for his operation before he can speak to her. On Ric’s request, Abra goes to change into scrubs so that he can look on during the surgery. As he changes we see a huge scar across his back.

Ric operates on Cassidy and after telling Michael he doesn’t need his assistance, an emergency occurs as he discovers a retroperitoneal hematoma, which bursts. With some persuasion from Ric, Abra jumps in and saves Cassidy. Lola and Jayne are not impressed with Ric’s actions, but Jayne agrees to give Abra one more chance and hires him again.

Lady Byrne and Elliot have been preoccupied trying to arrange a charity fundraiser. After coming up with endless inappropriate ideas, Elliot finally finds one, which is perfect, delighting Lady Byrne. In a moment of excitement and spontaneity, Lady Byrne and Elliot kiss briefly just as Connie enters the room. They are embarrassed and Connie leaves grinning, silently.

Alex decides to tell Simon everything and explains that she had a child when very young, but gave her away. She had given birth to her on the floor of a public toilet and called her Rosie. Maria listens from across the room and becomes emotional and leaves. She does however realise this is not the right time to tell Alex who she is and so just leaves her a single rose with a note saying “Your daughter would be proud”.

After bonding with Kyla, Daren cheers up even more when one of his foster brothers arrives. Kyla is shocked to see it is her son, Max. Kyla is delighted to see him but he keeps his distance. Kyla tries to question them about their foster mum but Max doesn’t want to talk and just says she’s a good person and better than she was to him. Kyla is hurt and Daren becomes upset saying that everyone he meets who is nice, always ends up leaving. Max up and leaves but is clearly in pain. Kyla, knowing he is diabetic, worries he is not eating or taking his medication properly, but can do nothing. Max later collapses in the stairwell and Kyla panics when she and Michael find Max having a fit on the floor.
